At Plan My Way, we provide a comprehensive range of services designed to support individuals with disabilities in achieving their personal goals and enhancing their quality of life. Our offerings are tailored to meet the unique needs of each participant, ensuring that they receive the most effective and personalised care possible.
Support Coordination & Psychosocial Recovery Coaching
At Plan My Way, we deliver expert Support Coordination across Levels 1–3, as well as tailored Recovery Coaching for participants with psychosocial disability.
What are the levels of Support Coordination?

Level 1: Support Connection
This foundational level is designed to help you understand your NDIS plan, connect with providers, and begin building your support network. It’s ideal for those who need light-touch assistance to get started and stay on track. -
Level 2: Coordination of Supports
This level provides more in-depth assistance, including building your capacity to maintain relationships, manage service delivery, resolve issues, and work toward your goals. Your coordinator helps coordinate multiple supports and ensures services remain responsive to your needs. -
Level 3: Specialist Support Coordination
This is a high-level, intensive support provided by experienced professionals for participants with complex needs or high risk circumstances. It involves working through barriers, crisis planning, and ensuring services are safely and sustainably implemented in challenging environments.
Our coordinators take the time to understand each individual’s needs, strengths, and aspirations. We assist with navigating the NDIS, building informal supports, coordinating service providers, and addressing barriers to progress.
We work closely with participants and their circles of support to ensure services are person-centred, goal-oriented, and adaptable to changing life circumstances. Whether you’re plan-managed, self-managed, or privately funded, we aim to simplify the system and help you feel empowered and supported throughout your journey.
This service is available under NDIS funding, and we also welcome privately funded participants or referrals through other community and government funding streams.

Housing Exploration and Design
Finding a place to call home is deeply personal. We help individuals explore housing options that align with their needs, preferences, and goals.

Our team provides practical, evidence-based support—including Housing Plans and applications for Specialist Disability Accommodation (SDA). We work closely with allied health professionals, support services, and housing providers to create clear, achievable housing pathways.
​
We have experience supporting people with complex needs including justice involvement, child protection, and hospital discharge planning—ensuring every person receives the right support, at the right time.
​
This service is available through NDIS funding (agency-managed, plan-managed, or self-managed), as well as through private or co-funded arrangements.

Private or non-NDIS clients may benefit from:

Navigating Centrelink or social housing systems

Finding or maintaining safe housing

Connecting with social workers or community support
-
Linking with financial counsellors, mental health teams, or disability advocates

Structured planning or service coordination during periods of transition

This support may be called “transition support” or “case management” for non-NDIS clients.
Finding and Keeping Employment
We believe everyone has the right to meaningful work. Our employment-focused support helps participants explore their interests and strengths, build job readiness skills, and connect with suitable training or job opportunities.

We assist with writing resumes, navigating Centrelink requirements, enrolling in employment programs, preparing for interviews and developing the confidence needed to pursue or sustain a job. For many, employment is also about social connection and personal growth—and we’re here for all of it.

This service can be delivered under NDIS (e.g., School Leaver Employment Supports or Finding and Keeping a Job funding), or accessed privately. We also collaborate with other services and funding bodies supporting disability employment pathways.


Daily Planning and Budgeting
Life feels more manageable when there’s a plan. Our team supports participants in developing skills to manage their time, appointments, routines, and money. We work together to create practical strategies that promote independence and reduce overwhelm.
Supports may include goal setting, breaking tasks into manageable steps, learning budgeting tools, using planners or assistive tech, and linking participants to ongoing supports when needed.
This service is designed to grow with the person and can be delivered through NDIS or private arrangements. It’s ideal for teens, young adults, or those transitioning from full support to independent living.
Life Stage Transition
Big changes can be exciting—and scary. We’re here to walk alongside participants as they navigate key life stages. Whether it’s moving from school to adult life, transitioning from care, navigating new roles in the community, or exploring a move toward independence, we provide thoughtful, goal-driven support.
Our approach is flexible and future-focused. We collaborate with families, mainstream services, and other professionals to support preparation, planning, and life skill development tailored to the individual’s vision for the future.
While often funded under NDIS, we also welcome private clients—including those who may be ineligible for the NDIS but require transitional support. This service works well in early intervention, youth justice, or housing exit pathways.
